SLMC Health Posts is a network of community-based clinics committed to bringing quality, affordable healthcare closer to communities. We offer essential medical services, outreach, education, and stigma-free sexual and reproductive health services for young people.

HP4Youth Digital Wellness Card
A simple way for young people aged 15–24 in the communities we serve to stay connected to confidential, judgement-free care. Your card helps our team keep your care together, so each visit builds on the last.
Register for free at any SLMC Health Post, at an HP4Youth Café event, or through partner Vocational Training Centres, schools and youth forums. Already have a card? Visit your nearest Health Post to upgrade it.
How to use your card
- Get your digital-enabled physical card through one of our registration points.
- Bring it to an SLMC Health Post or HP4Youth outreach event.
- Clinic staff scan its QR code through the authorised clinic system to retrieve your record and continue your care.
Free registration applies to the card; ask the Health Post about charges for individual services.

Good to know
Can I scan the card with my phone?
An ordinary phone or QR scanner cannot access your patient information. The QR code does not publicly display medical details; authorised personnel access the linked record through the clinic system.
What if I lose my card?
Report it to your nearest SLMC Health Post. The team can help replace or reissue the card and reconnect it to your existing record.
